Abstract

Background: Panic disorder may be accompanied by other psychological disorders during a chronic period without a timely diagnosis and treatment. Due to the similarity of symptoms in panic disorder and heart disease, it is neglected, resulting in social and economic costs in undiagnosed cases.

Objectives: To assess this disorder, we aimed to investigate the prevalence of panic disorder in patients with cardiac symptoms of noncardiac origin.

Methods: In a cross-sectional study with a descriptive approach, 80 patients with cardiac symptoms referred to the heart clinic of the Kowsar Hospital affiliated with Semnan University of Medical Sciences, entered the study in 2019 - 2020 by a convenience sampling method. Demographic and underlying data, including age, gender, education, smoking, marital status, hypertension, and history of heart disease in family members, were recorded. The evaluation of panic disorder was carried out using the "self-evaluation of panic disorder" questionnaire.

Results: The prevalence of panic disorder among patients was 10%, and all patients with panic disorder reported experiencing palpitations, while 75% reported chest pain. There is a significant association between the prevalence of panic disorder and the occurrence of palpitations (P < 0.001). Conversely, no significant relationship was observed between the prevalence of panic disorder and either chest pain (P = 0.704) or dizziness (P = 0.999).

Conclusion: Our findings suggested that all patients with panic disorder reported experiencing heart palpitations. This issue requires serious consideration, and patients with symptoms of non-cardiac origin need to be referred to psychiatry.
