Abstract
Background:
occupational accidents are one of the most important risk factors in industrial community which has adverse effects on economy, society and health. It should be prevented by some systematic methods and finding corrective measures. This study used 5WH technique for systematic assessment of accidents in steel company for 4 years.Material and methods:
the research was performed in workshops of steel company to analyze accidents for 4 years.
5WH determines how, what, why, where and when of an accident. This method can identify fundamental weaknesses of safety system and their relation with unsafe acts and conditions.
Results:
in total there were 130 recorded accidents. The highest frequency rate was in casting in 2005,
2006 and 2007, but it was in Eskelet felezy workshop in 2008. The effect of age, work experience, education and work shift assessed. Physical agents (noise and illumination) were surveyed too. The main cause of accidents was unsafe acts.
