Cancer was rarely diagnosed before the late 20th century. Nowadays, it is one of the leading causes of death and the most expensive disease with more complicated etiology and management. Human lifestyle and behavior are major risk factors and predicting causes of cancer.

Infection organism, an inappropriate diet, obesity, metabolic syndromes, drugs and chemical agents, and even psychosocial and spiritual behavior are effective elements to change the cell cycle and proliferation indexes, life or death of cells leading to cancer.

This complicated biology of cancer needs a complicated system to care for disease and patients.

Managers and caregivers must consider the fact that numerous specialist and complicated health delivery system should be at work in cancer care.

The aged people are more often target of cancer and eventually end up with more co-morbid condition. The younger people and children with this diagnosis are different communities with different needs. Definition of cancer cases and their quality are tremendously different because of economical status and the level of knowledge on the managers’ side in each country.

The cancer care should be patient centered, evidence based, comprehensive, and coordinated.

None of us would ignore the importance and necessity of cancer care. Hopefully, I will write about these complicated issues in the future.
